Flexible care options for patients in California and Alaska, including insurance, private-pay visits, and membership-based care.
Health Insurance & Coverage
Be Well Medical Group is in-network with select insurance plans. Coverage varies by state, service type, and whether you are a new or established patient.
Alaska
We continue to accept insurance for eligible Alaska patients, including Alaska Medicaid, Aetna, Curative, and Moda Health, including Moda Select, Connexus, Endeavor Select, Pioneer, and Tier 1 plans.
California
Be Well Medical Group is in-network with Optum and Aetna for behavioral health services, and with Curative for both medical and behavioral health services.
For patients with other medical insurance plans, we may be able to provide out-of-network care. Patients seeking ongoing primary care may also choose our Direct Primary Care membership for $150/month.
Our Direct Care Membership may also be used for eligible mental health and other ongoing medical services when appropriate.
New Patient Evaluations:
Extended Evaluations – 60 Minutes ($400)
Comprehensive initial evaluation for more complex concerns, including mental health, hormone care, multiple medical issues, or care that requires extra time and planning.
Standard Evaluations: 30-40 Minutes ($300)
Initial visit for focused concerns, general medical care, medication review, preventive care, or starting care when a full 60-minute visit is not needed. Available by telehealth or in person when appropriate
Ongoing Care: Choose from your options:
Pay-Per-Visit
No membership required
This option is best for patients who need occasional care, periodic follow-up, or help with a specific concern.
- 20–30 minute visit: $200
- 30–40 minute visit: $300
- 40–60 minute visit: $400
Visit length is based on the complexity of your concerns, the amount of care needed, and the time required for evaluation, planning, and documentation.
Best for:
- one-time concerns
- periodic medication management
- stable chronic conditions
- patients who do not need monthly visits
- patients who prefer flexibility
Many patients do well with follow-up visits every few months once their care plan is stable.
$150/month
Membership care is available for established patients who need more consistent support and ongoing medical management.
This option may be helpful for patients who benefit from regular follow-up, care coordination, and closer monitoring.

- routine follow-up visits
- chronic condition management
- medication management and adjustments
- hormone therapy support
- weight loss follow-up
- preventive care and general health support
- access to discounted lab and imaging rates
- In-office procedures (small fee may apply based on requires supplies and/or medications)
Membership is best for patients who need regular, ongoing care — not for patients who only need occasional visits.
Additional or extended visits may have added costs depending on the time and complexity of care required.
Direct Care Members receive 50% off additional or extended visit fees when:
- a visit requires up to 60 minutes rather than the standard 30-minute follow-up, or
- an additional visit is needed beyond the member’s routine monthly follow-up.
Direct Care Members also receive a $20 discount on our laboratory panels.
Important Membership Notes
Membership plans are not insurance and do not replace health insurance coverage.
Membership cannot be used together with insurance billing. Due to federal regulations and insurance contract restrictions, membership services are not available to Medicare patients or to patients whose insurance plan we are contracted with.
Medications are filled separately through pharmacies.
Prescription medications, laboratory testing, imaging, specialist care, emergency care, and hospital services are separate unless specifically stated otherwise.
Services Not Included in Membership
Addiction Medicine
Addiction medicine services, including treatment for opioid use disorder and alcohol use disorder, are not included in the membership program and are billed separately on a fee-for-service basis.
Because these services often involve greater clinical complexity, more frequent monitoring, refill coordination, safety planning, documentation, and additional clinical oversight, they are provided through a separate care structure rather than through the monthly membership.
Patients seeking addiction medicine care may still be seen on a private-pay or insurance basis, when clinically appropriate.
Fee-for-Service Rates for Addiction Medicine Services:
- 20–30 minute visit: $200
- 30–40 minute visit: $300
- 40–60 minute visit: $400
ADHD Evaluations
Initial ADHD evaluations are not included in membership because of the time and assessment required.
Initial ADHD Evaluation: $700
The initial evaluation includes an extended assessment. ADHD diagnosis may require more than one visit when clinically appropriate.
Ongoing ADHD follow-up may be available through private-pay or membership care depending on clinical needs, visit length, medication monitoring, and applicable regulations.
Affordable Labs & Diagnostic Imaging
We work with laboratory and diagnostic partners to help patients access competitive self-pay pricing when appropriate.
Direct Care members receive an additional $20 discount on the laboratory panels listed below.
