Why Direct Primary Care?
Modern primary care is often built around short visits, high patient volume, and addressing one concern at a time. That can make it harder for a primary care clinician to step back, connect the pieces, and manage multiple aspects of a patient’s health together.
The result can be fragmented care: one clinician for blood pressure, another for hormones, another for diabetes, another for mental health, another for weight management, and still another for sexual health — with no single person consistently looking at the whole picture.
We provide longer visits and broader management within primary care when clinically appropriate, helping centralize your care and reduce unnecessary fragmentation. Specialist care remains important when it is truly needed, but many concerns can be thoughtfully managed by a primary care clinician who knows your history, medications, goals, and overall health.
How Direct Primary Care Membership Works
Initial Evaluation
New patients begin with a comprehensive evaluation based on medical complexity:
40-minute evaluation — $300
Appropriate for many general primary care concerns and less complex medical histories.
60-minute evaluation — $400
Recommended for more medically complex care and required for:
- New mental health evaluations
- New hormone replacement therapy evaluations
Certain highly complex evaluations are not included in the Direct Primary Care membership model, including:
- ADHD evaluations
- Addiction Medicine evaluations
These services require additional assessment and follow-up beyond the scope of the standard membership structure.
Ongoing Membership — $150/month
After establishing care, members receive:
- Up to one 30-minute follow-up visit each month
- Visits that may be used for general medical care or mental health medication management
- In-person care in Beverly Hills or telehealth throughout California
- In-office procedures when appropriate, small fee for administered medications or supplies.
- Enhanced appointment access, including appointment availability reserved specifically for Direct Primary Care members
- Ongoing preventive care, chronic condition management, medication management, lab monitoring, and care coordination
- Need more than one visit in a month? Additional clinically appropriate visits are available at 50% off our standard fee schedule.
Unused monthly visits do not need to become the focus of the program — the value is having ongoing access and continuity when care is needed

Easier Access, With Time for Real Care
When a new symptom, medication change, or treatment decision needs attention, we prefer to make time for an actual conversation. That gives us the chance to understand what changed, review the bigger picture, discuss options, and make thoughtful decisions together.
Secure messaging is still available for brief questions, clarifications, and care coordination. But complex medical concerns are best handled in a dedicated visit, where we can give them the attention they deserve.
Member access includes:
- Additional visits at 50% off our standard fee schedule
- Priority appointment availability
- Prolonged 30-minute follow-up visits
- In-person care in Beverly Hills
- Telehealth throughout California
- Secure messaging for appropriate brief questions and coordination
Annual Comprehensive Visit
After 12 months of continuous membership, Direct Primary Care members receive one 60-minute comprehensive visit each year in addition to their regular monthly follow-up benefit.
This longer visit gives us time to step back and look at the bigger picture — reviewing your health history, medications, laboratory trends, preventive care, chronic conditions, mental health, sexual and hormone health, and goals for the year ahead.
Depending on your needs, your annual comprehensive visit may instead be used for a comprehensive psychiatric evaluation or hormone evaluation when clinically appropriate.
ADHD and Addiction Medicine evaluations remain outside the standard membership structure because of the additional time and complexity involved.
Direct Primary Care Is Not Health Insurance
Direct Primary Care changes how you receive and pay for care within our practice, but it does not replace health insurance.
Many patients choose to pair Direct Primary Care with a high-deductible or lower-premium PPO plan. Depending on the plan and the patient’s healthcare needs, this can provide strong protection for major medical expenses while making routine primary care more predictable and accessible.
Your PPO plan can generally still be used for eligible services outside our practice, including:
- Laboratory testing
- Diagnostic imaging
- Prescription medications
- Specialist care
- Emergency and hospital care
- Surgery and other covered services
Even when Be Well Medical Group is not billing your insurance for routine primary care, that does not necessarily prevent you from using your insurance benefits elsewhere. Coverage and out-of-network rules vary by plan, so patients should confirm their specific benefits when needed.

Helping Keep Healthcare Costs Manageable
We also understand that insurance does not always mean affordable.
We work with laboratory and imaging partners that may offer competitive self-pay pricing when that is more cost-effective than using insurance, and we have experience helping patients navigate care when they are uninsured, underinsured, or enrolled in high-deductible plans.
When appropriate, we can help identify:
- Lower-cost laboratory options
- Competitive cash pricing for imaging
- Generic and lower-cost medication alternatives
- Pharmacy discount programs
- Cost-conscious referral options
- When using insurance versus self-pay may make more financial sense
The goal is to help you use the healthcare system strategically — using insurance when it benefits you, and lower-cost alternatives when they make more sense.
